Approval rules are set per staff member, with limits by amount. You decide who can initiate a payment, who can approve one, and above what value approval becomes mandatory.
Why the limit matters more than the rule
A limit is a decision you make once, calmly, in advance. An approval request is a decision made repeatedly under time pressure. Set limits that match each person's actual role and you will need far fewer approvals.
The record
Every approval is recorded against the person who gave it and the time they gave it. That trail exists whether or not anyone ever asks for it, which is the point.
